With rapidly and adversely expanding economic impacts, uncertainties and risks that have engulfed our community and the nation with the dangerous Coronavirus pandemic, my medical practice’s operational and functional aspects are of no exception. Therefore, I have to announce the closure of my private medical practice effective April 30, 2020.
During the past 30 years, I have been truly honored to work with all of the many dedicated physicians, professionals, and all of the hard working staff who have been a part of our superb team, to deliver the highest quality of personalized fertility services to the South Florida community and to clients from all over the world. Most importantly, and with my deepest gratitude, I want to thank all of the patients who have allowed me to serve them, to provide medical care for them, and to help them through their journey of expanding their family through reproductive endocrinology. I thank you all from the bottom of my heart.
Please note that you are entitled to a copy of your medical records, and for medical records requests before April 30, 2020 (including transfers of the records to another medical provider or picking up a copy of the records yourself), please call the office directly at the number below to make arrangements. For medical records requests after April 30, 2020, please e-mail a scanned copy of your requests (found here) to: Requests@FertilityAndGenetics.com, and we will make arrangements with you at that time.
