Options at CARE-LIFE
Our philosophy is to provide highly
individualized medical treatment. We offer innovative and unique financial
options to our patients for the same high-quality medical services,
regardless of the financial program the patient chooses.
There are several financial options for IVF. These programs are designed
to accommodate each couple’s financial situation. In addition
to the routine one-cycle fee schedule, we also provide risk-sharing
programs, such as our Refund Programs, which have been developed specifically
to meet each individual couple’s needs and options.
The least expensive option for one-cycle of IVF treatment can be as
low as $7,990. There are two programs whereby the fees are based on
one treatment cycle, these are IVF CARE and IVF CHOICE. These options
may be combined with an exclusive membership program, only available
through our center. This membership program, or Care Advantage
Program will save 25% on all CARE and LIFE professional services.
Our risk sharing programs, involve multiple attempts. We offer two
IVF Refund Programs, one with medications and one without medications.
We also offer a multiple cycle package program called the IVF
Advantage Program, this can be associated with the lowest cost per treatment
cycle.
In order to determine which program is more advantageous to you, we
recommend that you request this information at the office, or by mail
or fax. Once you have reviewed this material we will be happy to answer
any questions you may have.
The following summarizes outstanding features of some of our innovative
financial programs.
1. IVF CARE Program *
This is our one-cycle IVF fee schedule. The entire cost for undergoing
one fresh IVF treatment at our center is $7,990. This includes
all related professional services for controlled ovarian stimulation,
oocyte retrieval, sonographic-guided embryo transfer (CARE); andrology
and embryology services (LIFE); anesthesia and facility fees.
2. IVF Choice Program *
This is a unique program whereby the fees for 3 fresh cycles are on
a sliding scale. If a live birth is not achieved following the first
attempt ($9,900), a significant savings of 40% can be realized per
cycle for the next 2 attempts. This has been a popular program and,
when combined with the CARE-Advantage Program (please see below) even
greater savings are possible.
3. IVF Advantage Program
*
This program provides up to 3 fresh IVF attempts including Laser Assisted
Hatching, Embryo Cryopreservation / Storage for up to one year, and
the use of all frozen embryos for a one set fee of $18,391. If no live
birth is achieved following the first cycle, then the second and the
third, if necessary, as well as transfer attempts for the cryopreserved
embryos obtained in these cycles, will be performed at no additional
fees*. This program is a popular choice among couples with more complex
clinical settings.
4. IVF Refund Program *
For qualifying patients this program provides up to 3 fresh cycles
and the use of all cryopreserved embryos. Fees encompassing C.A.R.E.-L.I.F.E.
professional services will be refunded ($10,000) for those without
a live birth. Based on the couple’s clinical criteria, a
total fee of either $21,830 or $23,830 will be assessed. Laboratory
procedures such as Laser Assisted Hatching, Embryo Cryopreservation,
and Storage for up to one year are included. If ICSI is indicated,
a fee of $4,000 applies for all ICSI procedures with a 50% refund
if no live birth is achieved. Please contact our office for full

5. New IVF Refund Program
with Medications Included**
This exceptional refund program provides qualifying couples up to 2
fresh IVF cycles and the use of all cryopreserved embryos. It includes
all injectable medications for controlled ovarian stimulation, blood
hormone monitoring tests while on stimulation, as well as ICSI, Assisted
Hatching, Cryopreservation and Storage of frozen embryos for up to
one year. If a live birth does not occur within one year, 50% of the
fees ($26,830) encompassing C.A.R.E.-L.I.F.E. professional services
will be refunded. Please contact our office for full details and qualification
criteria.
6. Ovum Recipient Choice
Program *
This is a unique program whereby the fees for 2 fresh attempts are
on a sliding scale. If a live birth does not follow the first attempt
($15,980), and the use of the cryopreserved embryos, there will be
a $2000 savings for the second attempt.
7. Ovum Recipient Refund
Program *
This is a unique program available for couples requesting in-vitro
fertilization with donor oocytes. This includes up to 2 fresh attempts
and the use of all cryopreserved embryos. If a live birth is not achieved,
the entire portion of the fees encompassing C.A.R.E.-L.I.F.E. professional
services will be refunded. Please contact our office for full details

8. CARE-Advantage Program
*
This unique membership program provides a 25% reduction for all C.A.R.E.-L.I.F.E.
professional services, including all A.R.T. services, surgical procedures
and diagnostic tests. This program may be used in conjunction with
the IVF CARE, IVF Choice and Ovum Recipient Choice Programs.
